QbCheck Screening is a paid test in the amount $150. Please be aware that at the completion of this form, you will receive a payment link to complete testing order placement.

Upon confirmation of the payment, you will receive a testing code with a link to complete the QbCheck Screening.

Upon completing the test, you will receive your comprehensive test result, and the result will be sent to your referring provider upon request.
